Optical Plan
The Optical Reimbursement Request Form should be completed and submitted with an attached receipt to the Treasurer as soon as possible after services are rendered.
The PBA Optical Plan is a self insured plan solely administered by the PBA. Each PBA membered is entitled to two hundred dollars ($200.00) of optical coverage per calendar year.
Spouses and dependent children up to age 19 are entitled to $200.00 of optical coverage per calendar year each.
Only one immediate household member is allowed one (1) pair of sunglasses per calendar year.
Dependent children over age 19 up to age 26 will receive the above benefit only if they are enrolled in college as a full-time student.
